Dapagliflozin Started During Heart Failure Hospitalisation: DAPA ACT HF-TIMI 68

  • What it examined: DAPA ACT HF-TIMI 68 tested in-hospital initiation of dapagliflozin 10 mg daily in 2,401 patients hospitalised for heart failure.
  • Key point: At two months, the trial primary composite of cardiovascular death or worsening heart failure was not significantly reduced (hazard ratio 0.86; 95% CI 0.68-1.08). In the prespecified meta-analysis of randomised in-hospital SGLT2 inhibitor trials, the composite was reduced (HR 0.71; 95% CI 0.54-0.93).

Why it matters

The study distinguishes an individual trial result from the wider randomised evidence base. Treatment decisions remain individual clinical decisions, including assessment of blood pressure, kidney function and contraindications.
